How are you tracking Ultimatum? You're not getting full use of it, leaving some free HS/Cleaves on the table, how could this improve?
I just went and looked through the logs I linked, and the only fight I see at a glance was Stone Guard, which can be explained by a poorly made macro involving Deadly Calm and it being listed first. I fixed that and now also have a weak aura for Ultimatum created. I also now have a weak aura to track Avatar being off cooldown, hooray.
I see you've glphed Death from Above, how are you using your Heroic Leap?
I don't always use Death from Above, actually. I generally use it in five-mans for the additional damage and movement (I generally like to pull entire rooms in heroics, if possible!), or if I'll need the extra movement 30 seconds earlier. I usually run with Endless Rage, Heavy Repercussions, and Hold the Line.
How are you using your shield barrier? Whats a case when you're popping a measly 20 rage vs. when do you pool for the big daddy 60 rage version?
I'm using shield barrier, as I explained above, when I'm dealing with attacks that aren't melee. The breath on Elegon, for example, which is weaved in between shield blocks for the melee. Lei Shei, if I recall, has only magical and no physical, which would warrant full shield barrier. As for 20 rage versus 60 rage, I believe Theck on Sacred Duty made some fancy graphs for us before the expansion hit, and 60 rage barrier are almost always preferred. When I use barrier on Stone Guard, they're all 60 rage barriers. I think the only times where you should use smaller barriers over the larger ones is when you're weaving it in with shield block and you just don't have the rage.
Do you have a means for tracking the Weakened Blows debuff? Or are you just counting on the other tank keeping it up?
I have a weak aura for it. I keep track of it and will refresh if I'm the one tanking or it's about to expire, but the other tank generally refreshes it well before it's close to expiring. In the case of Stone Guard, when I taunt a dog over, it already has the debuff, so I wait until it's about to expire before refreshing it. I don't believe in wasting a global for it, as that could be a devastate that yields a sword and board proc.
Why the parry/stam gems?
I itemize with balance in mind, because I feel the method of ignoring set bonuses and blindly stacking stam from the days of old is silly. In this case, the socket bonuses were stamina and strength, which I feel are beneficial stats and stupid to ignore. If it were dodge, I'd forgo the socket bonus.

They sat you for Spritbinder!?!? Madness. How did you approach that with your GM/RL. That's the worst decision they could make for that encounter how could you turn that around and get the better class in there to do the job?
